          Check-in for teams will be at the field where a team will play it’s first game in a competition, League or Tournament. The team representative is asked to be present for check-in 90 minutes prior to the team’s first game.

PRIOR TO FINAL REGISTRATION AND CHECK-IN

1. Once you have completed the player registration process:
          
> Send an e-mail to Registrar@FrontRangeFutsal.com notifying the Registrar that you have completed player registration. The Registrar will print Member Passes and a USSF Team Roster.
          
>Mail $6 check per youth player to: Registrar, Front Range Futsal League, POB 17344, Golden, CO 80402. (Be sure to identify the team on the check!) This USFF fee applies only to a player’s first FRFL competition in the 2005-2006 year. For example, a player who is first registered for the Winter League pays the USFF fee prior to participating in that competition. If that player subsequently participates in a FRFL tournament or the Summer League, there is no additional USFF fee.
          
In order to complete the preparation of Player Passes, this step should be completed at least 7 days prior to the start of the competition in which a team is entered.  

2. Print TWO copies of the Youth Player Registration form for each player. Both will need to be signed by the player’s parent or guardian. (One will go to the League at the time of check-in; the other you will keep.) If you are using a “free agent” as a guest, make a copy of that player’s “USFF Player Registration Form” (submit the copy, keep the original).
          
At Check-in a picture of each player will be required for the Player Pass. Proof of age for each player must be provided. Valid proofs of age are:
CS YSA Member Pass, Birth Certificate with a raised seal, valid Passport.

3. Print a copy of your team’s USFF Team Roster Form.

4. Download and complete the Competition Roster (Full Roster formor “Split Roster form” for League play, or the “Tournament Roster form”) . Your USSF Team Roster may contain as many as 20 players, but you may only use a maximum of 12 players in any game. (For more information: “Establishing Your Roster”.)
          
Have the players will sign these FRFL Competition Roster Forms.
          
After the rosters are signed, make as many copies of your Roster Form as you will have games in the competition. You will need to submit one copy to the scorekeepers before each game.

5. Consider having your team’s parents complete the “PCA Parent Pledge” developed by the “Positive Coaching Alliance” . The players, coaches and referees will greatly appreciate parents who honor the pledge through their words and actions.

AT CHECK-IN:

1. For the team, submit:
          
> The original of the Competition Roster for the competition (league or tournament)
          
> a copy of your team’s USFF Team Roster Form.

2. For each player, submit either:
          > A USFF Player Pass
(for those who have received passes for a prior 2004-2005 Futsal competition), or
          > A completed and signed Youth Player Registration form, along with valid proof of age are: CS YSA Member Pass, Birth Certificate with a raised seal, valid Passport.
          
If using a “Free Agent” guest player in a tournament who has not been registered with USFF, submit for each “Free Agent” player a completed and signed “USFF Player RegistrationForm” and the $6 USFF Player Fee. (See “ ‘Free Agent’ Player Registration” .)

3. You will receive Member Passes for players and coaches who have not received passes for a prior 2005-2006

FRFL competition. “Free agent” Member Passes will be issued on the spot. (Please have a picture ready.)

4. Before the first game of the competition, each player and coach must sign their respective Member Pass cards. The signature must be on the line above Player Signature on the back of the pass. A picture of each player and coach must be attached where the words “Attach Photo Here” is printed on the back of the pass.
